How to register

The practice is happy to accept patients living in the following areas:

  • Orrell,
  • Pemberton,

  • Upholland,
  • Whelley,
  • Winstanley,
  • Worsley Hall,
  • Worsley Mesnes, and any area within these boundaries.

You can register by completing a form available from reception. To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

New Patient Medical Examination

Each new patient joining the practice will be given the opportunity to undergo a medical check-up. This initial check-up will be performed by the Healthcare Assistant. This helps them to assess your general good health and to familiarise themselves with your medical history. This also applies to all children over the age of five years.
