Long Term Conditions – Annual Reviews

From the 1st April 2022 we are changing the way we recall patients for their Long Term Conditions Annual Reviews i.e. Asthma, COPD , Hypertension (BP) reviews. We will now be calling patients by birth month. For example, if you were born on 4th May 1968 and have COPD and Asthma, you will be recalled in May every year for your annual reviews. If you have more than one long term condition we are also aiming to do all reviews in one appointment slot. You’re appointment may be longer, but unless unwell or any problems, you will not need to be seen for another 12 months. This may mean that patients who may have only been into practice in February this year, may be called again before February next year.  Apologies for this
